Consent Form / Application Form

All players need to download and print their consent form (sometimes referred to as the Registration Form) and get it signed by a parent or guardian.

Once signed, please seal it in an envelope with the player's name on it. On arrival, pass this to the coach. Please do NOT forget the form because without it, you will NOT be allowed to take part.

To download the consent form, the parent/guardian will need to log into the Surrey Pathway Management System and look for the button marked 'Consent Form' at the bottom of the player's panel.
